Une initiative de modernisation doit avancer. Un programme de s\u00e9curit\u00e9 doit gagner en maturit\u00e9. Une nouvelle int\u00e9gration doit se faire. La premi\u00e8re question que se pose un\u00b7e dirigeant\u00b7e des technologies des soins de sant\u00e9 n\u2019est pas \u00ab\u202fqu\u2019est-ce que cela exige r\u00e9ellement\u202f\u00bb, mais \u00ab\u202fqui devons-nous embaucher\u202f\u00bb&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Cet instinct se comprend. L\u2019embauche est un levier familier. C\u2019est l\u2019outil vers lequel la plupart des organisations se tournent d\u2019abord, parce que c\u2019est celui que tout le monde sait d\u00e9j\u00e0 actionner. Toutefois, c\u2019est aussi la mauvaise question pour commencer, et y r\u00e9pondre trop t\u00f4t fait souvent perdre plus de temps qu\u2019on n\u2019en gagne.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong><strong><strong><strong><strong><strong><strong>Trois\u202flacunes diff\u00e9rentes, trois\u202fr\u00e9ponses diff\u00e9rentes<\/strong><\/strong><\/strong><\/strong><\/strong><\/strong><\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">La meilleure question est celle que la plupart des organisations sautent\u202f: de quel type de lacune s\u2019agit-il, au juste? S\u2019agit-il d\u2019une comp\u00e9tence pr\u00e9cise qui manque \u00e0 l\u2019\u00e9quipe? D\u2019un probl\u00e8me de capacit\u00e9 plus large, o\u00f9 l\u2019\u00e9quipe poss\u00e8de les bonnes comp\u00e9tences, mais pas assez de bras? Ou d\u2019un travail circonscrit qu\u2019il faut \u00e9valuer, planifier et livrer?<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Chacun de ces cas est un probl\u00e8me diff\u00e9rent. Chacun appelle une bonne r\u00e9ponse diff\u00e9rente. Aucun d\u2019entre eux ne se r\u00e8gle de fa\u00e7on fiable en se rabattant par d\u00e9faut sur une offre d\u2019emploi.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Nous voyons cela comme un continuum, et non comme un choix unique. Parfois, la bonne solution est un sp\u00e9cialiste\u202f: une personne poss\u00e9dant une comp\u00e9tence pr\u00e9cise, ajout\u00e9e \u00e0 une \u00e9quipe qui pilote d\u00e9j\u00e0 l\u2019initiative. Parfois, c\u2019est une petite \u00e9quipe de sp\u00e9cialistes aux comp\u00e9tences compl\u00e9mentaires, qui travaillent aux c\u00f4t\u00e9s des personnes d\u00e9j\u00e0 en place. Parfois, le travail est suffisamment bien d\u00e9fini pour \u00eatre cadr\u00e9 et livr\u00e9 comme un projet \u00e0 part enti\u00e8re\u202f: \u00e9valuer la situation, planifier l\u2019approche, l\u2019ex\u00e9cuter, la remettre \u00e0 l\u2019organisation.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">L\u2019erreur n\u2019est pas de choisir la mauvaise option parmi celles-ci. L\u2019erreur, c\u2019est de choisir avant d\u2019avoir nomm\u00e9 la lacune.&nbsp;<\/p>\n\n\n\n<div class=\"wp-block-group is-layout-constrained wp-block-group-is-layout-constrained\" style=\"margin-bottom:var(--wp--preset--spacing--50)\">\n<h2 class=\"wp-block-heading\"><strong><strong><strong><strong><strong><strong><strong>\u00c0 quoi ressemblait le fait de nommer la lacune sur une plateforme vieille de 15\u202fans<\/strong><\/strong><\/strong><\/strong><\/strong><\/strong><\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Un mandat de modernisation dans le secteur des soins de sant\u00e9 le montre bien\u202f: une organisation de soins de sant\u00e9 r\u00e9gionale devait migrer vers Azure une plateforme sur mesure vieille de 15\u202fans. La plateforme comportait une dette technique bien r\u00e9elle et une exposition tout aussi r\u00e9elle aux risques de s\u00e9curit\u00e9, le genre de probl\u00e8me qui figure sur tous les registres des risques et y reste pendant des ann\u00e9es, parce que personne n\u2019a de moyen simple d\u2019y rem\u00e9dier sans perturber ce qu\u2019elle prend en charge.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Dans ce cas-ci, les flux de travail qui reposaient sur cette plateforme ne pouvaient pas \u00eatre mis hors ligne. L\u2019\u00e9tablissement de soumissions, la comparaison de r\u00e9gimes, la g\u00e9n\u00e9ration de documents\u202f: tout devait continuer de fonctionner pendant que le syst\u00e8me sous-jacent changeait.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Dans une situation comme celle-l\u00e0, l\u2019instinct est souvent d\u2019ajouter du personnel et d\u2019esp\u00e9rer que l\u2019\u00e9quipe s\u2019en sortira en plus de tout ce qu\u2019elle a d\u00e9j\u00e0 \u00e0 faire. Ce qui s\u2019est produit \u00e9tait diff\u00e9rent\u202f: une \u00e9valuation d\u2019abord, pour comprendre exactement ce qui fonctionnait et ce qui en d\u00e9pendait, suivie d\u2019une migration et d\u2019une stabilisation s\u00e9quenc\u00e9es pr\u00e9cis\u00e9ment pour prot\u00e9ger ces flux de travail en production. Les travaux ont \u00e9t\u00e9 men\u00e9s \u00e0 terme, et la plateforme a \u00e9t\u00e9 enti\u00e8rement stabilis\u00e9e en 90\u202fjours, les flux de travail essentiels \u00e0 l\u2019entreprise demeurant en service tout au long du processus.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Rien de tout cela n\u2019a commenc\u00e9 par une d\u00e9cision d\u2019embauche. Tout a commenc\u00e9 par une question sur la nature du travail \u00e0 accomplir.&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong><strong><strong><strong><strong><strong><strong>Les programmes de s\u00e9curit\u00e9 suivent le m\u00eame sch\u00e9ma<\/strong><\/strong><\/strong><\/strong><\/strong><\/strong><\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Les enjeux li\u00e9s \u00e0 une ex\u00e9cution r\u00e9ussie augmentent, et non l\u2019inverse. En date de juin\u202f2026, 772\u202fatteintes de grande ampleur \u00e0 la protection des donn\u00e9es de soins de sant\u00e9 pour 2025 figuraient sur le portail des atteintes du U.S. Department of Health and Human Services, Office for Civil Rights (HHS\u202fOCR), selon l\u2019analyse des donn\u00e9es du portail r\u00e9alis\u00e9e par le HIPAA Journal. Ce nombre placerait 2025 au-dessus du sommet pr\u00e9c\u00e9dent de 746\u202fatteintes recens\u00e9es sur le portail en 2023. L\u2019analyse du portail montre que la plupart \u00e9taient li\u00e9es au piratage ou \u00e0 d\u2019autres incidents informatiques, et non \u00e0 des documents papier perdus. Ce n\u2019est pas une raison de s\u2019alarmer. C\u2019est une raison d\u2019\u00eatre pr\u00e9cis quant \u00e0 l\u2019endroit o\u00f9 se situe l\u2019exposition r\u00e9elle, et quant au type de travail requis pour y rem\u00e9dier. Un programme de s\u00e9curit\u00e9 qui doit gagner rapidement en maturit\u00e9 apr\u00e8s l\u2019arriv\u00e9e d\u2019un\u00b7e nouveau\u00b7elle dirigeant\u00b7e, ou apr\u00e8s un incident, ne manque pas, en g\u00e9n\u00e9ral, d\u2019un seul r\u00f4le. Il manque le plus souvent de capacit\u00e9 coordonn\u00e9e dans plusieurs disciplines \u00e0 la fois, soit l\u2019architecture, la gouvernance, la s\u00e9curit\u00e9 infonuagique et les risques li\u00e9s aux tiers, r\u00e9unie de fa\u00e7on d\u00e9lib\u00e9r\u00e9e plut\u00f4t que combl\u00e9e demande par demande.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">C\u2019est le m\u00eame sch\u00e9ma qui se r\u00e9p\u00e8te\u202f: nommer la forme r\u00e9elle de la lacune avant de d\u00e9cider comment y rem\u00e9dier.<\/p>\n<\/div>\n\n\n\n<h2 class=\"wp-block-heading\"><strong><strong><strong><strong><strong><strong><strong>Avant d\u2019ouvrir la demande<\/strong><\/strong><\/strong><\/strong><\/strong><\/strong><\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Rien de tout cela ne constitue un argument contre l\u2019embauche. Parfois, la bonne r\u00e9ponse est bel et bien une personne pr\u00e9cise poss\u00e9dant une comp\u00e9tence pr\u00e9cise, et rien ne la remplace. Le fait de traiter chaque lacune technologique comme un probl\u00e8me de dotation, par r\u00e9flexe, avant de comprendre en quoi elle consiste, revient toutefois, pour les organisations de soins de sant\u00e9, \u00e0 r\u00e9soudre le probl\u00e8me en fonction de l\u2019outil qu\u2019elles connaissent plut\u00f4t qu\u2019en fonction du probl\u00e8me qu\u2019elles ont.<\/p>\n\n\n\n<p
